    As part of the innovations in plastic surgery, a new technique has been developed to eliminate wrinkles without the pain of surgery.

    Many patients swear by the effectiveness of Botox or Restylane but their big problem was the pain. This is why Dr. Mark Steven Nestor, a dermatologist, recommends the use of a new anesthetic called Pliaglis.

    Pliaglis was approved just recently by the FDA. It is an anesthetic that combines high concentrations of two commonly used topical anesthetics. When it's spread on, it forms a gel-like mask that releases the anesthesia.

    "Because of amount of anesthetic in there and because of this occlusive aspect because of this gel or mask that it forms, it is very very powerful and it works very well," said Dr. Nestor.

    Thirty minutes after application, the Pliaglis is peeled away but the numbness stays. This way, the prick of the needle is almost pain-free.

    "The patients feel that this is virtually painless and it makes the procedure for them much much more pleasant," according to Dr. Nestor.

    You guys are way out of date!

    As of September 15, 2008, Galderma discontinued sale of Pliaglis.

    Galderma took this action because of recurring inconsistent viscosity (thickness) of the product formulation. Company testing found significant levels of variability in the viscosity parameter, resulting in a thicker cream which can make application of the product difficult.

    I loved Pliaglis, and I hope that it is re-released. It was safe and effective, but it sure was a pain to apply.

    While it is true that injectable treatments like BOTOX and wrinkle fillers) can be painful, the use of topical anesthetics to dull the pain is hardly news.

    I asked my friend, a plastic surgeon in Naperville whose wide is also a dermatologist, whether they had heard about this. He kinda laughed a little and then told me that it has been common, as long as there have been injectables and lasers, for doctors to offer topical anesthetics for patients who were really sensitive to the pain. It sounds like the topical treatment often impacts the price though. And I know I read somewhere that some laser treatments can be impacted by topical anesthetic gels.

    It seems like it would be a good idea to talk with your doc about how sensitive you are to pain, what options there are to help alleviate that discomfort, how it impacts the cost of treatment, if there are any risks, and if the presence of the anesthesia can impact the outcome of the treatment.
